Step 2: After you've received your equipment, please fill out the form - including serial number

Step 3: Schedule and complete installation

Step 4: Receive your rebate check in approximately 60 days after installation

Promotion valid on eligible intraoral scanners purchased October 1, 2023 - December 31, 2023. Rebate claims must be submitted post installation and requested by March 31, 2024. Please allow up to approximately 60 days post-install and validation of qualifying new purchase for rebate. This promotion cannot be combined with any other offers. Rebate checks to be issued in USD currency and mailed to the individual person or company named on the form submitted, which must match the name of the purchaser of the DEXIS Intraoral Scanner. Only one check can be received per eligible scanner serial number.

End User Rebate Claim Information: To claim your end user rebate, go to https://go.dexis.com/rebate, complete the form and click the “CLAIM REBATE” button. Only one rebate for each qualified product purchased will be eligible. Rebates are available to end users only. Purchasers who intend to sell, distribute, assign or otherwise transfer the product are not eligible. End users must strictly comply with the eligibility requirements and all terms and conditions of the program. The rebate will be paid in the form of check in USD currency.
